Procedures for Issuance of Confirmation Form

For Those who Need to Obtain the “Confirmation Form”

For those who applied for the issuance of a Certificate of Eligibility before June 22, you will be required to submit the following two documents in addition to the regular visa application documents. 

  1. Confirmation on foreign nationals regarding entry into Japan under the New Border Measures (27), etc./ Use of Fast Track and Visit Japan Web
    (“Form for Applicant”)
  2. Confirmation on foreign nationals regarding entry into Japan under the New Border Measures (27), etc./ Use of Fast Track and Visit Japan Web
    (“Form for Host Institution”)

Please follow the procedures below.

  1. Please read the following pages carefully to understand Fast Track and Visit Japan Web.

  2. Fill out “Form for Applicant”and submit it by email to  OU_entryJP●adm.okayama-u.ac.jp.
    *Please replace “●” with “@” in the above email address.
    *Please append your name in alphabet to the beginning of the file name.

    Please follow the guidance in STEP 1for the rest of the immigration procedures.

  3. After completing the procedures in STEP 1, you will receive the “Form for Host Institution” by email together with the “Certificate for Completion of Registration to the ERFS system”.
  4. When applying for a visa, please submit above documents “Form for Applicant” and “Form for Host Institution” to Japanese embassies / consulates in addition to the regular visa application documents.
